This Privacy Policy sets out the rules for the collection, processing and protection of personal data provided by you in connection with your use of the www.hairmitage.pl website, use of services through the www.harimtiage.pl website and use of services at Hairmitage facilities.

In the interests of the security of the data entrusted to us, we have developed internal procedures to prevent disclosure of data to unauthorized persons. We control their performance and constantly check their compliance with the relevant legal provisions.
Personal data is processed on the basis of your consent and in cases where the law authorizes the Administrator to process personal data on the basis of legal provisions or to implement the contract concluded between you and Hairmitage.

The data provided in the forms are processed for purposes resulting from the function of a specific form, e.g. to contact you or to provide you with information.
In addition, we collect the following personal data for the purposes set out below:
- information on medical history, health, medications taken, therapies, age, height, body weight to ensure the safety of the services provided
- name, surname, telephone number, e-mail address in order to be able to contact and transfer information
- name, surname, PESEL number, home address to be able to issue a prescription
- name, surname, address, telephone number and e-mail address of third parties authorized to receive information about you and confirmation of the possibility of using this data by an authorized person in order to be able to provide information to such third parties by your authorized person.
Your personal data, including those left on the website www.hairmitage.pl, will not be sold or made available to unauthorized third parties. Personal data and data regarding the current and historical health condition may be made available only to entities authorized to obtain them on the basis of legal provisions and entities participating in the provision of services. Personal data will not be transferred to a third country or an international organization.
You have the right to access your personal data and the right to rectify, delete, limit processing, the right to transfer data, the right to object and the right to withdraw consent to data processing. Withdrawal of consent does not affect the lawfulness of data processing, which was made on the basis of consent before its withdrawal.
Providing personal data is voluntary, however, failure to provide them may prevent the proper performance of services or the refusal to perform them.
Your personal data will be kept for the period required by law.
Your data will not be processed in an automated manner, including in the form of profiling, i.e. no decisions that have legal effects on a person or in a similar way significantly affect it will not be based solely on the automatic processing of personal data and are not automatically associated with decision made.
We reserve the right to change the rules of this Privacy Policy, including with regard to the use of the website www.hairmitage.pl, which may result from the development of internet technology, the development of the Hairmitage website, changes in the scope of services provided by Hairmitage, changes in the protection law personal data and other important premises. We will inform you about any changes in a visible and understandable way.
The www.hairmitage.pl website may contain links to other websites. Such websites operate independently of the www.hairmitage.pl website and are in no way supervised by the www.hairmitage.pl website. These websites may have their own privacy policies and regulations, which we recommend that you read.

- Act of 15 July 2011 on the professions of nurse and midwife
- Act of April 15, 2011 on medical activity

- Your personal data is processed in order to protect the facility and property and ensure security on the premises of the Administrator. Video surveillance is carried out as part of the Administrator's legitimate interest.
- Data processing using the video monitoring system includes entrances to the Administrator's facility, passageways inside the Administrator's facility, and the reception hall area of the Administrator's facility.
- Your personal data may be made available only to entities authorized to obtain it on the basis of legal provisions.
- Your personal data collected through video surveillance will not be stored for more than one month.
- Your personal data will not be transferred to a third country or an international organization.
- You have the right to access your data and the right to rectify, delete, limit processing, the right to transfer data, the right to object and the right to withdraw consent to data processing. Withdrawal of consent does not affect the lawfulness of data processing, which was made on the basis of consent before its withdrawal.
- You have the right to lodge a complaint with the supervisory body - the Personal Data Protection Office, if you feel that the processing of your personal data violates the provisions of the General Data Protection Regulation of April 27, 2016.
- Your data will not be processed in an automated manner, including in the form of profiling, i.e. no decisions that have legal effects on a person or in a similar way significantly affect it will not be based solely on the automatic processing of personal data and are not related to an automatically made decision.
- These regulations define the terms and conditions of the Promotions announced on the Organiser's website.
- The organizer of the Promotion is Hairmitage Sp. z o.o. with headquarters in Warsaw (02-797) at ul. Klimczaka 22D/U3, entered into the Register of Entrepreneurs kept by the District Court for the Capital City of Warsaw in Warsaw, XIII Commercial Division of the National Court Register under the KRS number 0000801714, using the NIP number: 9512489693 and REGON number: 384273039.
- The aim of the Promotion is to popularize the Organiser's services.
- Promotions are carried out in the Organizer's facilities, limited in terms of the deadline for applications for participation, limited in duration, limited as to the subject, limited in terms of privileges (benefits) of participants and limited in terms of availability (number).
- The Organizer shall each time specify the detailed terms of the Promotion on the Organiser's website www.hairmitage.pl, including in particular:
- subject of the Promotion (scope of services)
- dates of applying to take advantage of the Promotion
- Promotion dates, including the start and end dates of the Promotion
- privileges (benefits) of people benefiting from the Promotion
- restrictions on the use of the Promotion
- the possibility of combining the Promotion with other Promotions carried out by the Organizer.
- Participation in the Promotion is voluntary, and participants use the Promotion at their own risk.
- Participation in the Promotion constitutes acceptance of the promotion rules contained in these Regulations and in the detailed terms of the Promotion published for each Promotion on the Organiser's website www.hairmitage.pl.
Promotion participants may be adults or minors with the appropriate consent of their legal guardian, who are consumers within the meaning of the relevant provisions of the Civil Code. - Promotion participants may be persons whose health and current condition do not preclude participation in the Promotion for specific services. The organizer informs about contraindications to the provision of services on the website www.hairmitage.pl/uslugi/ in the tab regarding the promoted service. The Participant is obliged to read the contraindications before joining the Promotion and to inform the Organizer about circumstances that constitute a contraindication to participation in the Promotion.
- Applying for the Promotion is possible through:
- booking the service / services covered by the Promotion by contacting the Organizer at +48 690 799 007 or
- booking services / services covered by the Promotion by direct contact with an employee of the Organizer and
payment in accordance with the promotional price list announced by the Organizer. - The use of the Promotion takes place through the provision of the service / services covered by the Promotion in accordance with the conditions specified by the Organizer. The Organizer is not responsible for improper use of the Promotion, in particular for failure to provide all or part of the services covered by the Promotion and the resulting damage, if it was for reasons not attributable to the Organizer.
- The promotion participant may use the promotion any number of times, subject to the provisions of point 5 of these Regulations.
- The privileges (benefits) resulting from the Promotion cannot be exchanged for a cash or material equivalent.
- Personal data of Promotion participants will be used based on applicable law, in particular Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC and the Act of 10 May 2018 on the protection of personal data.
- Any complaints regarding the Promotion may be submitted by e-mail to the following e-mail address: clinika@hairmitage.pl or by calling +48 690 799 007. Complaints will be processed within 14 days from the date of their receipt by the Promotion Organizer. The complaint procedure adopted by the Promotion organizer does not in any way violate the rights granted to the Promotion participant under generally applicable law, including the possibility of pursuing claims through court proceedings.
- The Organizer bears no responsibility for non-compliance by the participants of the Promotion with the rules set out in these Regulations.
- The Organizer of the Promotion reserves the right to change the Regulations in the event of an important reason understood as:
- change in the law regulating the rules and organization of the Promotion, affecting the mutual rights and obligations of the Organizer and Promotion participants
- extending or shortening the term of the Promotion
- change in the manner of conducting the Promotion due to technical or technological reasons
- other important reason not attributable to the Organizer
- other reason important from the point of view of the legitimate interest of the Organizer.

- In the event of a change in the Regulations, the Organizer will make the consolidated text of the Regulations available by publishing it on the Organiser's website www.hairmitage.pl. The amendment to the Regulations does not affect orders placed as part of the Promotion before the change is made.
- The generally applicable provisions of Polish law shall apply to all matters not covered by these Regulations.
